Please bring your insurance card, photo identification and co-pay for your visit.  If you printed and filled out paperwork from the website, please bring that as well.

Also, bring any imaging performed and a medication list, if applicable.

For your first clinic appointment, you will change into a gown for a physical exam, so it is best to wear clothing that is easy to change in and out of.  

All Injections/Procedures

We ask that you arrange to be driven to and from the appointment.  While we do not anticipate side effects for most patients, some will have dizziness and other symptoms after the procedure that will interfere with driving. 

Thoracic and Lumbar Procedures

​

Please wear clothing that is loose and comfortable.  Pants should have a stretch waistband.

Cervical Procedures

​

You will change into a gown for the procedure.
